{"id":3296,"date":"2024-04-22T16:11:08","date_gmt":"2024-04-22T10:41:08","guid":{"rendered":"https:\/\/www.edhacare.com\/blogs\/?p=3296"},"modified":"2024-05-08T11:50:24","modified_gmt":"2024-05-08T06:20:24","slug":"chemotherapy-for-acute-lymphocytic-leukemia","status":"publish","type":"post","link":"https:\/\/www.edhacare.com\/blogs\/chemotherapy-for-acute-lymphocytic-leukemia\/","title":{"rendered":"Chemotherapy for Acute Lymphocytic Leukemia","gt_translate_keys":[{"key":"rendered","format":"text"}]},"content":{"rendered":"<p><span style=\"font-weight: 400;\">Acute lymphocytic leukemia (ALL) is also called acute lymphoblastic leukemia. \u201cAcute\u201d means that the leukemia can progress quickly and, if not treated, can be fatal within a few months. &#8220;Lymphocytic&#8221; means it develops from early (immature) forms of lymphocytes, a type of white blood cell.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">ALL starts in the bone marrow (the soft inner part of certain bones, where new blood cells are made). Most often, the leukemia cells invade the blood fairly quickly. They can also sometimes spread to other parts of the body, including the lymph nodes, liver, spleen, central nervous system (brain and spinal cord), and testicles (in males). Some cancers can also start in these organs and then spread to the bone marrow, but these cancers are not leukemia.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Other types of cancer that start in lymphocytes are known as lymphomas (either non-Hodgkin lymphoma or Hodgkin lymphoma). While leukemias like ALL mainly affect the bone marrow and the blood, lymphomas mainly affect the lymph nodes or other organs (but may also involve the bone marrow). Sometimes it can be hard to tell if a cancer of lymphocytes is a leukemia or a lymphoma. Lymph tissue is found in lymph nodes, the thymus, the spleen, the tonsils, and adenoids and is scattered throughout the digestive and respiratory systems and the bone marrow.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Lymphocytes develop from cells called lymphoblasts to become mature, infection-fighting cells. There are 2 main types of lymphocytes:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">B lymphocytes (B cells): B cells help protect the body by making proteins called antibodies. The antibodies attach to germs (bacteria, viruses, and fungi) in the body, which helps the immune system destroy them.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">T lymphocytes (T cells): There are several types of T cells, each with a special job. Some T cells can destroy germs directly, while others play a role in either boosting or slowing the activity of other immune system cells.<\/span><\/li>\n<\/ul>\n<p><a href=\"https:\/\/www.edhacare.com\/treatments\/hematology\/lukemia-treatment\"><span style=\"font-weight: 400;\">Acute lymphocytic leukemia<\/span><\/a><b> develops from early forms of lymphocytes<\/b><span style=\"font-weight: 400;\">. This can start in either early B cells or T cells at different stages of maturity.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The American Cancer Society\u2019s estimates for acute lymphocytic leukemia (ALL) in the United States for 2024 (including both children and adults) are:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">About 6,550 new cases of ALL (3,590 in males and 2,960 in females)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">About 1,330 deaths from ALL (640 in males and 690 in females)<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">The risk for developing\u00a0 Acute lymphocytic leukemia L is highest in children younger than 5 years of age. The risk then declines slowly until the mid-20s and begins to rise again slowly after age 50. Overall, about 4 of every 10 cases of ALL are in adults.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Subtypes of Acute Lymphocytic Leukemia (ALL)<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The World Health Organization (WHO) system, most recently updated in 2016, includes some of these factors to try to better classify ALL. The WHO system divides ALL into several groups:<\/span><\/p>\n<p><b>B-cell <\/b><span style=\"font-weight: 400;\">Acute Lymphocytic Leukemia\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">B-cell ALL with certain genetic abnormalities (gene or chromosome changes)<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">B-cell ALL with hypo-diploidy (the leukemia cells have fewer than 44 chromosomes [normal cells have 46])<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">B-cell ALL with hyperdiploidy (the leukemia cells have more than 50 chromosomes)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">B-cell ALL with a translocation between chromosomes 9 and 22 [t(9;22)] (the Philadelphia chromosome, which creates the <\/span><i><span style=\"font-weight: 400;\">BCR-ABL1<\/span><\/i><span style=\"font-weight: 400;\"> fusion gene)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">B-cell ALL with a translocation between chromosome 11 and another chromosome<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">B-cell ALL with a translocation between chromosomes 12 and 21 [t(12;21)]<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">B-cell ALL with a translocation between chromosomes 1 and 19 [t (1;19)]<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">B-cell ALL with a translocation between chromosomes 5 and 14 [t(5;14)]<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">B-cell ALL with amplification (too many copies) of a portion of chromosome 21 (iAMP21)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">B-cell ALL with translocations involving certain tyrosine kinases or cytokine receptors (also known as \u201cBCR-ABL1-like ALL\u201d)<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">\u00a0<\/span><span style=\"font-weight: 400;\">B-cell ALL, not otherwise specified<\/span><\/p>\n<p>T-cell\u00a0 <span style=\"font-weight: 400;\">Acute Lymphocytic Leukemia\u00a0<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Early T-cell precursor lymphoblastic leukemia<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">\u00a0<\/span><span style=\"font-weight: 400;\">Acute lymphocytic leukemia <\/span><span style=\"font-weight: 400;\">\u00a0is not a common cancer, accounting for less than half of 1% of all cancers in the United States. The average person\u2019s lifetime risk of getting ALL is about 1 in 1,000. The risk is slightly higher in males than in females and higher in white people than in African Americans.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Most signs and symptoms of <\/span><span style=\"font-weight: 400;\">acute lymphocytic leukemia <\/span><span style=\"font-weight: 400;\">\u00a0are the result of shortages of normal blood cells, which happen when the leukemia cells crowd out the normal blood-making cells in the bone marrow. These shortages show up on blood tests, but they can also cause symptoms, including:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Feeling tired<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Feeling weak<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Feeling dizzy or lightheaded<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Shortness of breath<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Pale skin<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Infections that don\u2019t go away or keep coming back<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Bruises (or small red or purple spots) on the skin<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Bleeding, such as frequent or severe nosebleeds, bleeding gums, or heavy menstrual bleeding in women<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Patients with <\/span><span style=\"font-weight: 400;\">acute lymphocytic leukemia <\/span><span style=\"font-weight: 400;\">\u00a0also often have several non-specific symptoms. These can include:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Weight loss<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Fever<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Night sweats<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Loss of appetite<\/span><\/li>\n<\/ul>\n<h3><span class=\"ez-toc-section\" id=\"general_symptoms\"><\/span><b>General symptoms<\/b><span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p><b>Swelling in the abdomen:<\/b> <span style=\"font-weight: 400;\">Leukemia cells may build up in the liver and spleen, making them larger. This might be noticed as fullness, swelling of the belly, or feeling <\/span><span style=\"font-weight: 400;\">full after eating only a small amount.\u00a0<\/span><\/p>\n<p><b>Enlarged lymph nodes:<\/b> <span style=\"font-weight: 400;\">ALL that has spread to lymph nodes close to the surface of the body (such as on the sides of the neck, in the groin, or in underarm areas) might be noticed as lumps under the skin.\u00a0<\/span><\/p>\n<p><b>Bone or joint pain: <\/b><span style=\"font-weight: 400;\">Sometimes leukemia cells build up near the surface of the bone or inside the joint, which can lead to bone or joint pain<\/span><\/p>\n<p><b>Spread to other organs<\/b><\/p>\n<p><span style=\"font-weight: 400;\">\u00a0 \u00a0 Less often, ALL spreads to other organs:\u00a0<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">If <\/span><span style=\"font-weight: 400;\">Acute lymphocytic leukemia <\/span><span style=\"font-weight: 400;\">\u00a0spreads to the brain and spinal cord, it can cause headaches, weakness, seizures, vomiting, trouble with balance, facial muscle weakness or numbness, or blurred vision.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">\u00a0It may spread inside the chest, where it can cause fluid buildup and trouble breathing.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Rarely, it\u00a0 may spread to the skin, eyes, testicles, ovaries, kidneys, or other organs<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Symptoms from an enlarged thymus<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u00a0\u00a0The<\/span><b> T-cell subtype of <\/b><span style=\"font-weight: 400;\">Acute lymphocytic leukemia <\/span><b>\u00a0<\/b><span style=\"font-weight: 400;\">\u00a0often affects the thymus, which is a small organ in the middle of the chest behind the sternum (breastbone) and in front of the trachea (windpipe).\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The superior vena cava (SVC), a large vein that carries blood from the head and arms back to the heart, passes next to the thymus. If the thymus is enlarged, it may press on the SVC, causing the blood to \u201cback up\u201d in the veins. This is known as SVC syndrome. The best way to find leukemia early is to report any possible signs or symptoms of leukemia .\u00a0 .Some people are known to have a higher risk of acute<\/span><span style=\"font-weight: 400;\"> lymphocytic leukemia<\/span><span style=\"font-weight: 400;\">\u00a0(or other leukemias) due to genetic disorders like Down syndrome or because they were previously treated with certain chemotherapy drugs or radiation. Most doctors recommend that these people have careful, regular medical checkups. The risk of leukemia, although greater than in the general population, is still very low for most of these people.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Certain signs and symptoms can suggest that a person might have acute lymphocytic leukemia (ALL), but tests are needed to confirm the diagnosis.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Medical history and physical examination<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Tests used to diagnose and classify ALL<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Blood tests<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Blood chemistry tests<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Complete blood count (CBC) and Peripheral blood smear<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Bone marrow tests<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Bone marrow aspiration\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Bone marrow biopsy\u00a0<\/span><\/li>\n<\/ul>\n<p><b>Lab tests: <\/b><span style=\"font-weight: 400;\">One or more of the following lab tests may be done on the samples to diagnose AML and\/or to determine the specific subtype of ALL.<\/span><\/p>\n<p><b>Routine exams with a microscope<\/b><\/p>\n<p><span style=\"font-weight: 400;\">Determining what percentage of cells in the bone marrow are blasts is particularly important. A diagnosis of ALL generally requires that at least 20% of the cells in the bone marrow are blasts. Under normal circumstances, blasts don&#8217;t make up more than 5% of bone marrow cells.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Sometimes just counting and looking at the cells doesn\u2019t provide a definite diagnosis, and other lab tests are needed.<\/span><\/p>\n<p><b>Cytochemistry: It entails utilizing a variety of methods to localize the chemical components of cells and cell organelles on thin histological sections.<\/b><\/p>\n<p><b>Flow cytometry and immunohistochemistry: <\/b><b>Flow cytometry is a technique that examines individual cells suspended in a solution to determine their dimensions, form, and different surface markers.
